What trimix is
Trimix is an injectable solution containing three medications — papaverine, phentolamine and alprostadil — given as a small injection into the side of the penis. It works locally on the blood vessels and smooth muscle of the penis rather than through the bloodstream, which is why providers often turn to it when oral tablets have not worked or are not suitable.
Related formulations:
- Bimix contains papaverine and phentolamine, without alprostadil. Sometimes used when alprostadil causes discomfort.
- Quadmix adds atropine to the three trimix components.
- Alprostadil alone is an option for patients who need to avoid papaverine.
These are compounded preparations. They are not FDA-approved drug products, and the FDA has not evaluated them for safety, efficacy or quality. Your provider selects the formulation and the dose.

Concentrations are per mL. Papaverine is held at 30 mg/mL across the standard blends, with alprostadil and phentolamine varying.

Additional formulations available on request. The full menu runs from alprostadil 5.9 mcg/mL up to 300 mcg/mL, with and without atropine. If your provider writes a concentration not listed here, call us — we can almost always match it, usually with next-business-day turnaround.
Why trimix is not covered by insurance
Medications for erectile dysfunction are excluded from Medicare Part D by statute, and most commercial plans exclude compounded preparations because they do not carry an NDC number. Trimix is a cash-pay medication almost everywhere.
The number that matters is cost per use, not cost per vial. At typical injection volumes, a single vial holds a large number of doses. Call us with your prescribed volume and we will run the math against what you would pay per tablet.

Pickup or mail. You can pick up at the pharmacy, or we ship within Minnesota and Wisconsin. First fills are best done in person, we will walk you through storage, measuring your dose and technique before you go home.
Storage — read this before your first dose
Trimix arrives frozen and should be kept frozen until first use. After you have drawn your first dose, store it in the refrigerator.
Do not leave it out of cold storage for more than about three hours. If you are traveling, use an insulated case designed to keep medication cold. We stock them.
Phenylephrine, if your provider has prescribed it, is stored at room temperature.
Safety
- Priapism. If you get an erection lasting more than four hours, call your provider immediately or go to the emergency room. Do not wait it out. Untreated prolonged erections can cause permanent damage.
- Frequency. Follow your provider's limit on how often to inject. Injecting too frequently increases the risk of scarring.
- Site rotation. Alternate sides with each injection.
- Blood thinners. If you take aspirin, warfarin or another anticoagulant, you will need to hold pressure on the injection site longer. Talk to your provider before starting.
- Do not repeat a dose. If an injection does not work, do not inject again the same day.
- Not for everyone. Trimix is not appropriate if you have significant penile scarring, an active infection or sore on the penis, or an allergy to any component.

Can I get trimix without a prescription?
No. It is prescription-only. If you do not have a provider, a urologist or men's health clinic is the right place to start.
How long does a vial last?
It depends entirely on your prescribed volume and how often you inject. Call us with your prescription details and we will tell you.
What if my formulation is not listed?
Call us. We can source nearly any concentration from the same manufacturer, typically next business day.
Do you ship?
Yes, within Minnesota and Wisconsin. Pickup is a good option if you're nearby, trimix ships frozen, and picking it up means it goes straight from our freezer to yours.
